Our application is open to men and women ready to commit to our seven-month residential program in Sapulpa, Oklahoma. Applications may be completed by the individual seeking recovery, or with the help of a family member, case manager, or chaplain if needed.

Steps to Admission
Our admissions process is designed to be simple and supportive:
- Submit Application – Online, by mail, or via email.
- Interview – Staff will schedule a phone or in-person interview to confirm readiness and answer questions.
- Acceptance – Applicants are notified by phone or letter with an intake date or placed on a waitlist if full.
- Intake Day – On arrival, applicants complete orientation and paperwork. For details on what to bring, see our Preparing for Treatment page.
- Orientation – Participants meet staff and peers, are assigned a bed, and begin their recovery journey.
Referrals
Lifegate Freedom Recovery accepts referrals from courts, probation officers, ministries, and other recovery organizations. Professionals may use the same application form or contact our team directly for guidance. We frequently work with court-ordered participants, provided they are willing to commit voluntarily to the program.

After Applying
Applications are typically reviewed within a few business days. If you do not hear back within a week, we encourage you to reach out to confirm we received your form. While waiting for acceptance, we recommend attending church or local recovery meetings to stay focused on your goals.
